Incident Reporting Procedures


Incidents which must be considered for reporting include, but are not limited to:
  • Coach Ejection (Send Off)
  • Spectator Incident
  • Match abandonment for reasons other than Natural Causes.
If a report is to be filed, please utilize the following procedures:
  1. Do NOT contact UIL first. UIL filing may be completed after the local chapter representative has been contacted (see next steps)
  2. Evening of incident: Head official should contact the District Representative for the School or Venue. District Reps can be found at Here
  3. Evening of incident: Head Official should contact one of the following Lee Windham, Fred Hiler, Wayne Mazure, Marty Stringer. Official should continue calling until an actual person is reached. Leaving a voice mail is not acceptable.
  4. Within 24 hours: Head Official should contact other officials and notify them of procedures to be followed which should include the following:
  5. Within 24 hours: All officials should write reports and submit to the chapter representative that you discussed the report with. A copy should also be sent to Lee Windham.
  6. Within 48 hours: Specified Executive Team members review reports to ensure concise/factual reporting is made. Recommendation on how to proceed is determined and sent to crew.
  7. Within 72 hours: If executive team recommends UIL reporting, then All Officials are to submit reports to UIL using the form or instructions found at TASO. Reports submitted to UIL should match what had been submitted and reviewed by the executive committee.
